We focus on preventive measures, healthy habits, and sustainable lifestyle changes that improve well-being. Role Overview: As an In-Home Health Coach , you will meet with seniors to set wellness goals, develop action plans, and provide motivation to adopt healthier behaviors. Your personalized approach supports lasting changes that enhance physical, mental, and emotional health. Key Responsibilities: Assess clients’ health status, habits, and readiness for change. Create tailored wellness plans focusing on nutrition, exercise, and stress management. Monitor progress, adjust strategies, and celebrate successes with clients. Collaborate with healthcare providers and caregivers to ensure integrated support. Qualifications: Certification or background in health coaching, wellness, or related fields.
